  [RelLookupTableConverter][NFC] Hoist loop invariants in createRelLookupTable (#223557)

IntPtrTy, Int32Ty, and Base (ptrtoint of the relative lookup table) are
invariant across all elements of the lookup table. Hoist them out of the
element iteration loop to avoid redundant recomputations.
