I have changed how this PR works:

# Host
The Fortran interfaces now call into the C interfaces im `kmp_csupport.cpp` as trampolines to adjust the ABI differences. We might want to consider making these the C entry points and forward to an implementation routine in the `__kmp` namespace for an actual implementation. I will talk to @kevinsala at the OpenMP FTF to sketch an implementation beyond a mere `return NULL`.

# Device
`DeviceRTL` did not have Fortran interfaces yet. Since we are targeting Flang, I have added device entry points that act as similar trampolines as on the host, simply adjusting the ABI between Fortran and C. I did not go all the way with entry-point macros like we use them for host entry points. Device code is much more restricted in how it is generated, and thus this should not be too much of a restriction.
