LOG: Revert "[OpenMP] Implement TR8 `present` map type modifier in runtime (2/2)"

This reverts commit 45b8f7ec35ef653bafdf48034857222517c17781.

It attempts to use debug macros `DPxMOD` and `DPxPTR` in release
builds.  Will fix and reapply later.
