[PATCH] D24180: Emit 'no line' information for interesting 'orphan' instructions

I've identified one or two refinements that help some specific cases, but haven't had a chance to try to implement them yet. Also I haven't tried Reid's suggestion for seeing how CodeView behaves.  It is near the top of my list.
