[PATCH] Fix TableGen -gen-disassembler output for certain bit field definitions.

Changes from previous patch:
1) Use more succinct test file name "BitOffsetDecoder.td"
2) Update test file comments to use "offset" instead of "shift"

Verified all tests pass.
