Updates and improvements

- Adds a bunch of new headers to the std module.
- The module now uses the build headers instead of the system headers. This still needs more testing in external projects.
- Implement a way to mark unsupported tests.
- Update documenation, including with review comments.
- Add some work-arounds for issues in headers. Obviously they need to be reviewed in their own patches. (Several of them have patches by other libc++ maintiners.)
