[cfe-dev] Objective-C top-level constructs code generation

Hi,

I've started splitting the Objective-C code generation stuff up into  
smaller diffs.  This one is a bit bigger than the others will be, but  
the places where it touches existing code are fairly isolated so it  
should be relatively easy to review.

This provides implementations for the Objective-C classes, protocols  
and categories on the GNU runtime. I will include the implementation  
for the Étoilé runtime in a separate patch.
