N3891: A proposal to rename shared_mutex to shared_timed_mutex

Attached is a patch that implements N3891 which was recently approved at
the Issaquah meeting.

This patch was tested against x86_64-apple-darwin13.1.0
and x86_64-pc-linux-gnu with a svn checkout at r202407.
