[PATCH] clang-replace: Re-org of file structure

  Eventual inclusion into clang itself was a reason why I wanted to re-org the directories to mirror clang. Thinking about it overnight I think there needs to be an extra directory here: include/clang-replace/Replace/ instead of include/Replace. clang-tools-extra seemed like a good place to incubate this new library/tool.
