[PATCH] Add user documentation for cpp11-migrate

  I'd really like to avoid a proliferation of different Sphinx setups throughout LLVM. Could you move this up so that it is in clang-tools-extra/docs/ so that it can be shared with the other tools?

  Also in the discussions of risk you should both provide examples of situations where it will perform correct transformations *and* examples of incorrect transformations that it may perform (especially for "risky").
