Log:
Change ObjCRuntime GenerateProtocol[Ref] methods to take
    ObjCProtocolDecl directly.

Implement CodeGen support for forward protocol decls (no-ops are so
    nice to implement).

Also moved CGObjCRuntime.h out of CodeGenModule.h
