  Tooling: Migrate some tests to FileEntryRef, NFC

Migrate to the `FileEntryRef` overload of `SourceManager::createFileID`
(using `FileManager::getOptionalFileRef`) in RefactoringTest.cpp and
RewriterTestContext.h.

No functionality change.
