Log:
Rename LiveIntervalAnalysis.h to LiveIntervals.h

Headers/Implementation files should be named after the class they
declare/define.

Also eliminated an `#include "llvm/CodeGen/LiveIntervalAnalysis.h"` in
favor of `class LiveIntarvals;`
