[PATCH] Permit duplicate explicit class instantiations if MSVCCompat is enabled

Thanks Reid,

Committed as r208402. I went with "... ignored as a Microsoft extension"
which isn't perfect, but I can't think of an obvious alternative without a
complete rewrite.
